Why Chronological Age Matters in Occupational Therapy Assessment

Occupational therapy evaluations rely heavily on norm-referenced instruments. How to Calculate Chronological Age for OT Testing

The method is identical to the one used across clinical disciplines: subtract the date of birth from the date of assessment to get age in years, months, and days. Then apply the rounding convention your test manual specifies.

The Round-Down (Completed Months) Convention

Most standardized OT assessments use a round-down approach. You drop remaining days and report only completed years and months. This is written in years;months format with a semicolon separator.

Worked example:

  • Date of birth: October 22, 2019
  • Assessment date: June 5, 2025
  1. Years: 2025 − 2019 = 6, but June comes before October, so subtract 1 → 5 full years.
  2. Months: June (6) − October (10) + 12 = 8, but day 5 < day 22, so subtract 1 → 7 full months.
  3. Days: 5 − 22 + 31 (days in May) = 14 days.

Full chronological age: 5 years, 7 months, 14 days.

Using the round-down convention, the CA recorded on the score sheet is 5;7. The 14 remaining days are dropped because the child has not yet completed the 8th month.

Some instruments score by total completed months rather than years;months bands. In this example, that would be 67 months (5 × 12 + 7).
